In a traditional fee-for-service system, physician compensation depends on the number of surgical procedures performed and does not include a patient-based measure of utility such as incremental quality-adjusted life years (QALYs) [1]. Therefore, physicians may be unwilling to adopt a patient-oriented pricing approach. To overcome this problem, a transition to value-based purchasing (VBP) has been proposed in order to introduce patient-based measurements into the current physician reimbursement paradigm.
However, a major obstacle for the VBP initiative is the inability to accurately measure and compare QALY gains across different clinical specialty areas. This article is designed to address this issue by evaluating the relationship between current relative value unit (RVU)-based prices and patient utilities in the most common surgical procedures in South Korea. 으라차차정형외과
Using accessible Korean data, the study investigated the disparity between provider-based RVUs and consumer-based incremental QALYs in the most common surgical procedures and provides insight into how the current RVU based pricing system may influence physician job satisfaction and patient health outcomes.
Using the patient-based utility metric of incremental QALY, the results showed that RVUs and QALY gains exhibited a negative correlation. The relationship was more pronounced for fatal surgical procedures than non-fatal ones, suggesting that the current medical pricing system prioritizes costly and urgent surgeries at the expense of those with substantial health-related quality of life improvements. A subgroup analysis revealed that even within fatal procedures, the current price-utility relationship was still adverse.
